I'd add a fanatical protestantism to that list, I'm told anti-catholic intolerance ranks with a hatred of islam and juddhism in the US, sure the creation of "the american way" to try and create a universal culture was intrinsically protestant with it's preaching of work ethic and alcohol prohibition.
