     9        they do or whether they do not, but assume they do.  What
    10        do you make of this document as a procedure for ensuring
    11        that the right things happen when an accident occurs?
    12        A.  I think it is a very positive document.  It requires
    13        people to fill in the HEO's numbers, and so on.  It gives
    14        concise advice as to the categories of accidents to be
    15        reported, and it is intended to be displayed in the units.
    16        It seems a very positive document.

    18   Q.   If you turn -- I never know whether it is backwards or
    19        forwards -- to P, I think it is.  I am sorry for the
    20        quality of these copies.

    24        A.  Yes.

    33        A.  These are not, no.  These are internal forms.

    39        Do you regard the existence and, one must hope, the use of
    40        such forms as beneficial from a health and safety point of
    41        view, or not?
    42        A.  Yes.  I would regard it as good practice to report
    43        incidents.

    49        Colchester McDonald's Accident Book?

    52   Q.   I draw to your attention that that gives a figure of around 
    53        two accidents -- it may be RIDDOR as well -- per month over
    54        an eight year period.  Mrs. Barnes tells us she thinks that
    55        is rather low and that it would be more like eight, on
    56        average, throughout the operation.
    57        A.  She would have more specific knowledge of that than me.
